Unpaid balances, including all applicable co-payments, co-insurance, deductibles and any non-covered services are the responsibility of the patient and must be paid upon receipt of the statement. Payments may be made via

We will make available, under the Financial Assistance Program, a reasonable volume of service at no cost or less than full charges to persons meeting the criteria. Eligibility for financial assistance will be limited, on a first request basis, to persons whose family income is not more than the current poverty income guidelines established by the Department of Health and Human Services.
