Care Delivery Model

Fundamental to the healthcare practitioner in the delivery of patient care are the key domains of Professional Practice, and these form the basis for our Care Delivery Model.

These domains include:

• Culture/Caring
  The central theme in the St. Joseph Nursing Center of Excellence is the caring relationship that exists between the patient, family, and the nurse. Our Center's caring practices are responsive to the uniqueness of the patient/family and create a compassionate and therapeutic environment.

• Clinical Practice
  Our nurses collaborate with patients, family, and community to achieve mutually agreed-upon goals. They accomplish these goals by creating a collaborative and integrated approach to advanced care and advanced caring.

• Collaboration
  The entire healthcare team plans and practices together, within their individual scopes of practice. The team shared values and mutual respect for each other's contribution.
