Birth Certificate Requests

If you have given birth at Mission Hospital, we make it easy to obtain a birth certificate. All that is required is that you complete the form given to you by the Health Information Services to the County of Orange. This is then forwarded to the Social Security Administration, which then mails you the Social Security card.

Medical Records Requests

Looking to obtain a copy of your medical record? Under the State of California's Confidentiality of Medical Information Act, we require an Authorization for Disclosure of Health Information before releasing records to anyone. Patient Concierge

At Mission Hospital, we know that staying in a hospital can be stressful and we therefore work tirelessly to ensure that your visit is as stress-free as possible. For this reason, we are proud to offer our one-of-a-kind Concierge service that can help with everything from picking up items at home to paying bills and making hotel arrangements. Patient Financial Assistance

For many patients, money is tight. This, however, should not mean that you are not able to receive the medical attention that you deserve. By providing a Patient Financial Assistance Program, we are able to give eligible patients the assistance that they need to receive necessary medical attention. In some cases, those who are eligible don't pay at all; in other situations they only need make a partial payment.

To streamline the process and to ensure an efficient stay with us, we encourage patients to take advantage of our online pre-registration form. We encourage visitors to fill out this form three days before their stay with us for insurance verification purposes. We are also able to pre-register patients by phone and via fax.
