To speed up your admission into Mission Hospital, we have provided several convenient ways to pre-register you.
If you have any questions about our Online Pre-Admission Forms, please call our Admitting dept. at (949) 364-1400 ext. 7222 or send an email to missionadmitting@stjoe.org.
| Online Pre-Registration (recommended) |
Fill out the Online Pre-Registration Form and submit it directly over the Internet using our highly-secure system. It is recommended that this form be submitted at least three days prior to your expected admitting date for insurance verification purposes.
Once the Mission Admitting Department receives your pre-registration form, we will take care of your pre-registration process.

Download and complete the Adobe Acrobat Online Pre-Registration Form, and fax it to the Mission Admitting Department at
(949) 365-2306. It is recommended that you fax the registration for at least three days prior to your expected admitting date for insurance verification purposes.
Once the Mission Admitting Department receives your pre-registration form, we will take care of your pre-registration process.

You must mail your Pre-Registration Form at least two weeks prior to your expected admitting date for insurance verification purposes. Once the Mission Admitting Department receives your Pre-Registration Form, we will take care of your pre-registration process.

| Pre-Registration via Phone |
You can call the Mission Admitting Department at (949) 364-1400 ext. 7735 and ask one of our registrars to pre-register you over the phone. If a registrar is unavailable, please leave a voicemail and your call will be promptly returned. In your voicemail, please be sure to provide the following information:
• Your name
• Your phone number
• Best time to reach you
• Your Social Security Number (if you feel comfortable doing so)
